Rachel Novak, FNP

Dr. Chance-Lawson grew up in Dallas, Texas. She moved to Chicago for Medical School and obtained her medical degree from Chicago College of Osteopathic Medicine. She did her Family Medicine Residency at Cook County Hospital in Chicago, Illinois.

She decided to make the South East Wisconsin area her home sixteen years ago when she moved here with her husband and raised her 3 children in the area. They added the newest member of their family during covid, Milo, their dog.

Dr. Chance-Lawson enjoys the relationships she builds with her patients and emphasizing empowering her patients to play an active role in their healthcare.

As a life-long resident of Kenosha, Rachel believes in caring and treating those in her community. She believes in supporting and providing personalized and holistic care for her patients. She understands care should be patient focused with the patient at the center of decision making. Rachel sees that the best way to provide high-quality patient care is to acknowledge each patient's individuality and to meet them on their level. Health, wellness and illness should be viewed through the lens of the patient as a whole. Patient care should be comprehensive and compassionate with an emphasis on educating the patient while building a strong therapeutic relationship built on trust and best evidenced-based practice. The focus of this health care clinic will be to take care of patients of all ages with chronic health care conditions and healthy routine visits. The slogan” Keeping Families Healthy” is meant to provide comprehensive health care with expediency and stabilize chronic health conditions so that people stay healthy or become healthy.
